I am looking up prices for shipping. USPS has a box (Priority Mail Box O-BOX7). In the rates for shipping (4 ounces) with that box there is this: Priority Mail® Service(?) with 2-3 Day Service Standard priced at $4.80.

I click the (?) and it says

Does this mean the box, which is 12"X12"X8", can be shipped for $4.80 anywhere in the US as long as it is 70lbs or less? Or am I completely lost?

No, Each size of Flat Rate Box has its own rate. The first one you mention is the "Small" box and you can send it for a flat $4.80(if postage is printed online).

The 12x12x8 Flat Rate Box is $13.45(with online postage).

ONLY the boxes that are specifically marked as Small/Medium?Large Flat Rate Box are eligible for the flat rates of postage. All other USPS provided Priority Mail boxes will pay the rates based on weight and zone.

Go to the post office. It's a $4.80 envelope. It's free. They should have some on the wall if yours is open 24/7 or go ask for one. I've shipped w/ one before. It's very small. It will not hold 70 lbs. You can get a flat rate Priority box for $9.80 and that is the price for up to 20 lbs. I've only used those two before. Generally if it's something small I use a bubble mailer. The free Priority boxes are good for cd's, jackets, shirts, etc. Anything that takes up space. You can fit like 13 shirts in the $9.80 box, a great way to go! 70 lbs will cost like $35 at least! Try the eBay forums if you use that. Someone there will be able to tell you since people there do shipping all the time.

You should use the USPS websites shipping calculator:

http://postcalc.usps.gov/

There might be a money difference in price between what you put in (weight) and what the USPS scale reads. Be prepared for a little bit of price difference.

This is the Post Office's website. For $13.95 you can ship any package 70 pounds or less. The size is 12 1/4 X 12 1/4 X 6.

There are 2 smaller "flat rate boxes" as well.

These boxes are available for no charge at the Post Office.

Only if it is a priority mail flat rate shipping box is it a set price. I think you are talking about the priority box but not flat shipping. (you are gonna make me go to the site). Ok I looked it up and you are talking about regular priority not flat rate. Like I said depending on the weight. If you estimate the weight of the item you can click on calculate shipping on the website and it will tell you around what it will cost. Its says up to 70lbs because they won't take it if it is over that. I mail things all the time.

I brought a USPS priority box (12x12x8) to the post office with prepaid postage ($15.80) and lady there said that I have to pay additional postage because the box does not say large flat rate.

If your items weighs more than two pounds, it is probably cheaper to use the Priority box, which is not charged by weight. Any other box is charged by weight and size. Only if you are mailing books can you get a better rate.

To figure how much shipping is the post office 1) determines how far the box has to travel, and 2) weighs the box — although they do have flat-rate shipping, too. If your package is heavy, then get a flat-rate shipping box. It's one price no matter how heavy it is.

I think that's what it means, I know that they have the flat rate shipping boxes also, where its a flat rate to anywhere in the US. To make sure I'd either call UPS or your local post office and ask.
